I understand that dogs placed through the French Bulldog Rescue of Ontario have been acquired, fed and boarded, received medical care, inoculations, and have been spayed/neutered at the personal expense of the volunteers who comprise Rescue and who receive financial compensation only as it is available through donations I have answered all the questions on this application truthfully to the best of my knowledge and ability. I have read a copy of the adoption contract used by the FBRO, understand it and agree to abide by its terms should a French Bulldog be available for placement in my home. I understand that if, for any reason, I can no longer care for or keep the dog, I must return it directly to the FBRO, and I will sign all necessary papers for the surrender of the dog. I give my veterinary clinic permission to release to FBRO Executive any requested documents on pets they have attended to for me in the previous 3 years and this authorization is current for one year from application to adopt. I understand that by signing this form, I agree to release and covenant to hold harmless The French Bulldog Rescue of Ontario and its members from any claims, damages, costs, or actions incurred as a result of the care or actions of the animal. I accept full responsibility for the animal(s) actions at all times, and release the FBRO from any liabilities or damages that may be incurred as a result of adopting such animal(s). I hereby make application to the FBRO to have my name added to the waiting list for the purpose of giving a French Bulldog a permanent home. *I understand
